For depression, trazodone's label describes starting at 150 mg/day in divided doses and increasing by 50 mg/day every three to four days as tolerated, up to a maximum of 400 mg/day for most outpatients. For insomnia, off-label doses are typically much lower and are also usually started low and adjusted slowly.
