Trazodone Dosage Guide

A summary of typical trazodone dosing across its approved and off-label uses, drawn from FDA prescribing information.

Editorially reviewed
Key points
  • 150 mg/day starting dose for depression per FDA label
  • 400 mg/day usual outpatient maximum; 600 mg/day for select inpatients
  • Off-label sleep doses are typically much lower (25–100 mg)
  • Always individualized — never self-adjust dosing

For major depressive disorder, trazodone's FDA label describes a starting dose of 150 mg/day in divided doses, titrated up by 50 mg/day every 3 to 4 days as tolerated. The usual outpatient maximum is 400 mg/day, while more severely depressed inpatients may receive up to 600 mg/day under closer supervision.

For off-label insomnia use, typical doses are considerably lower — often 25 to 100 mg at bedtime — reflecting clinical experience rather than a formal FDA-reviewed dosing range for that indication.

Dosing should always be individualized by your prescriber based on your diagnosis, response, age, and other health factors. Never adjust your own dose without guidance.
