Trazodone and carbamazepine

Carbamazepine strongly induces the CYP3A4 enzyme that breaks down trazodone, which can significantly lower trazodone blood levels and reduce its effectiveness.

Key points
  • A strong CYP3A4 inducer
  • Can significantly reduce trazodone's effectiveness at a normal dose
  • May require trazodone dose adjustment

Carbamazepine, used for seizures and certain mood disorders, is a strong inducer of CYP3A4 — the liver enzyme mainly responsible for metabolizing trazodone. Combined use can substantially lower trazodone blood levels, potentially making a normal dose less effective. Prescribers may need to increase the trazodone dose or monitor response more closely.
