For each entry, record: date, time since your last dose, the symptom, its severity (mild/moderate/severe), how long it lasted, and anything else you were doing (e.g., just stood up, hadn't eaten, took another medication).
Patterns to watch for: symptoms that appear at a consistent time relative to your dose, symptoms that worsen after a dose increase, and any symptom from our emergency warning signs list — those need same-day medical attention, not just a diary entry.
